I agree to pay Tara McGillicuddy, Director of ADDClasses.com $227 for the 6 week program Adult ADD BootCamp - Organized and in Control which begins on 5/27/09 at 12:00 pm EASTERN (New York Time) .

I understand that Tara McGillicuddy, ADDClasses.com, and Linda Hillger are not responsible for any decisions I make after enrolling in Adult ADD BootCamp - Organized and in Control.

I understand that I must be over the age of 18.

I understand that I must use a personal organizer (or PDA during) Adult ADD BootCamp - Organized and in Control.

I understand that long distance telephone charges may apply. I understand that it's best to call in using a regular land line telephone and that cell phones and VOIP services may or may not be reliable.

I understand that I must be a registered member of ADDClasses.com in order to access the Adult ADD BootCamp - Organized and in Control class area.

I understand that in order to use the ADDClasses.com site I must have cookies enabled in my web browser.
